Thank you for reaching out to Volunteer Legal Services Hawaii regarding advice and counsel clinics. Before we can proceed with scheduling you for a clinic (based on volunteer attorney availability), you must complete, e-sign, and submit the following form:

VLSH Clinic Acknowledgement Form

Remember, you must complete the form and submit it to VLSH before any services can be scheduled. Please print out a copy of each form for your own records.
